This Notice describes how medical information about you may be used and disclosed by Sprout Anew Mental Health Services LLC and how you may obtain access to that information.
Our Responsibilities

How We May Use and Disclose Your Information
Treatment: To evaluate, diagnose, coordinate, and provide psychiatric or behavioral healthcare.
Payment: To bill health plans, verify coverage, obtain authorization, process claims, and collect collect amounts owed.
Healthcare operations: To support quality improvement, training, compliance, credentialing, auditing, business planning, and administrative functions.
Appointment reminders and communications: To contact you regarding appointments, treatment follow-up, portal messages, or practice services.
Business associates: To approved vendors that perform services for the practice and are contractually required to protect information when applicable.
Legal or public-interest purposes: When required or permitted by law, including certain reporting, oversight, judicial, law-enforcement, safety, workers’ compensation, or public-health activities.
Preventing serious harm: When permitted by law to reduce a serious and imminent threat to a person or the public.
Some uses and disclosures require your written authorization. You may revoke an authorization in writing, except where action has already been taken in reliance upon it.

- ✓ File a complaint without retaliation.
Additional protections may apply to psychotherapy notes, substance-use treatment records, minor records, reproductive-health information, and other specially protected information.
Complaints
You may submit a privacy complaint to Sprout Anew’s Privacy Officer or to the U.S. Department of Health and Human Services Office for Civil Rights. Sprout Anew will not retaliate against you for filing a complaint.
