Your Four-Step New-Patient Pathway

Getting started is simple. Follow these four steps to begin your care journey with Sprout Anew Mental Health Services LLC.

Request Care

Select “Become a New Patient” and complete the brief appointment-request form. Include the service you are seeking, telehealth or in-person preference, general availability, preferred contact method, and insurance or self-pay preference.

Important: Submitting a request does not guarantee an appointment or establish a provider-patient relationship.

Confirm Eligibility and Insurance

Our team may confirm your age, requested service, residence, physical location during telehealth, provider licensure and availability, outpatient clinical appropriateness, insurance benefits, and any referral or authorization requirements.

Important: Insurance verification is not a guarantee of payment.

Complete New-Patient Forms

After approval, you will receive secure patient-portal instructions. Complete registration, medical and psychiatric history, medications and allergies, insurance and billing information, treatment and telehealth consents, privacy acknowledgments, financial policies, and relevant screening questionnaires before the appointment.

Prepare for the First Appointment

Have a government-issued ID, insurance card, pharmacy information, medication list, allergies, relevant records, prior psychiatric information, provider contacts, and questions ready.

For telehealth, use a private, quiet, well-lit location with reliable internet and test your camera and microphone in advance.

BEFORE YOUR APPOINTMENT

Important information to help determine whether our services and secure telehealth appointments are appropriate for you.

Telehealth Eligibility

Telepsychiatry appointments are available only when clinically appropriate and legally permitted.
During each telehealth appointment, the patient must be physically located in a state where the treating clinician is licensed. Sprout Anew currently serves eligible patients located in Virginia, Maryland, and Washington, DC, subject to provider licensure, service availability, and applicable law.
At the beginning of each visit, patients may be asked to confirm identity, physical location, a reachable telephone number, emergency contact information, and whether they are in a safe and private setting.
Patients must not participate while driving or operating machinery. Telehealth may not be appropriate when emergency intervention, inpatient care, intensive monitoring, an in-person examination, or another level of care is required.

Age and Clinical Eligibility

Sprout Anew provides care to eligible adolescents and adults according to provider qualifications, service availability, and clinical needs.
For minors, a parent or legal guardian may need to provide consent, submit proof of guardianship, and attend part or all of the initial appointment. Confidentiality and record access will follow applicable law. Some conditions may require referral to an emergency department, hospital, specialized program, substance-use facility, or another provider.

Insurance and Payment

Sprout Anew may accept selected commercial insurance plans and other approved payment arrangements.
Before the first visit, submit insurance information, confirm the plan is active, determine whether psychiatric and telehealth services are covered, verify whether prior authorization or a referral is required, and review deductible, copayment, and coinsurance responsibilities.
Patients choosing self-pay will receive applicable fee information before services are provided. A Good Faith Estimate may be available to uninsured or self-pay patients under applicable federal requirements.

Appointment Requests and Scheduling

Appointment requests may be submitted online, by telephone, or through a secure message.
After receiving the request, the team will review eligibility, discuss the requested service, confirm provider availability, verify insurance information, provide portal and intake instructions, and schedule the appropriate appointment.
Do not submit urgent, highly sensitive, or emergency information through an ordinary website contact form.

Medication and Prescription Information

Patients should bring or securely upload a complete list of prescription medications, over-the-counter products, vitamins, and supplements.
Prescription decisions depend on a complete psychiatric evaluation, clinical judgment, applicable law, safety considerations, and professional standards. Requesting a particular medication does not guarantee that it will be prescribed.
Additional requirements may apply to controlled medications, stimulants, sedatives, and substance-use treatment, including prescription-monitoring review, identity verification, medical records, laboratory testing, toxicology screening, treatment agreements, periodic in-person assessments, or coordination with other clinicians.
Refill requests should be submitted through the secure patient portal sufficiently in advance of running out.

Spravato® and Specialized Services

Spravato® and other specialized treatments require a separate clinical assessment and may involve confirmation of diagnosis and treatment history, insurance authorization, medical clearance, in-person administration, post-treatment monitoring, transportation arrangements, and compliance with federal safety requirements.

Submitting an inquiry does not establish eligibility. A clinician will determine whether the treatment is clinically appropriate.

Cancellation and No-Show Expectations

Patients should provide the notice stated in the practice cancellation policy when changing or canceling an appointment.

Late cancellations and missed appointments may result in a fee not covered by insurance. Repeated missed appointments may affect continued scheduling.

The complete policy should be available in the patient portal and intake documents.

Confidentiality and Secure Communication

Patient forms, clinical messages, records, and treatment-related communications should be submitted through the secure patient portal or another approved encrypted system.

The general website contact form should be used only for basic appointment inquiries and should not contain detailed medical histories, diagnoses, Social Security numbers, identification documents, insurance documents, urgent medication concerns, or crisis messages.

Patients should review the Notice of Privacy Practices, Website Privacy Policy, and Telehealth Consent before beginning care.

Urgent and Emergency Needs

Sprout Anew is not an emergency or crisis-response service. Website forms, voicemail, portal messages, and appointment requests are not monitored continuously.
